Directions:

• From I-205 take Exit 16 for Johnson Creek Blvd.
• Head east up the hill one block.
• Turn right at SE 91st Ave.
• Take the second driveway on your left.
• Enter Mount Scott Professional Plaza II and take the elevator to the 4th floor.
• Turn left for suite 400.
• Check in at main desk at scheduled appointment time.

• From I-205, take the Johnson Creek Blvd - Exit 16.
• Then turn east (uphill) on Johnson Creek Blvd to SE 91st Avenue.
• Please arrive 30 minutes prior to your scheduled appointment time.
• Park by the main entrance and go directly to the Admitting Desk to check in.

• From I-5 take the Tualatin/Sherwood Exit 289.
• Turn left onto SW Nyberg Road
• SW Nyberg Road becomes SW 65th Ave.
• The hospital will be on your left
• Arrive at the hospital one hour prior to your scheduled appointment time.
• Park by the main entrance to the hospital and go directly to the Main Admitting desk to check in.
• They will direct you from there.

• From I-84 take the 58th Ave Exit.
• Turn Right onto NE Glisan St.
• The hospital will be on your right.
• After parking, please enter the Main Entrance of the hospital and follow the sign to the Main Admitting Desk.
• They will direct you from there.
• Please arrive 15 minutes before your scheduled appointment.

• From Highway 26 take the Barnes Road exit.
• Turn Right on Barnes Road; the hospital will be on your left.
• You can either valet park or park in the East Pavilion.
• Locate the gray elevators located near Vinnie’s café.
• Take the elevators to the lower level.
• Go down the hallway to the Medical Procedures Unit.
• You will check in here.
• Please arrive 30 minutes before your scheduled appointment.

• From US-26 take Exit 64 for 185th Ave.
• Follow signs for Hillsboro/Beaverton.
• Turn left at NW 185th Ave.
• Turn right at NW Evergreen Parkway.
• Turn left at NW 188th Ave.
• Turn left at NW Cornell Rd.
• Take a slight right to stay on Cornell Rd.
• The Surgery Center at Tanasbourne will be on your right.
• Check in at main desk.

• From US-26 take Exit 64 for 185th Ave.
• Follow signs for Hillsboro/Beaverton.
• Turn left at NW 185th Ave.
• Turn right at NW Evergreen Parkway.
• Turn left at NW 188th Ave.
• Turn left at NW Cornell Rd.
• Take a slight right to stay on Cornell Rd.
• The Providence Tanasbourne Health Center will be on your right.
• Take elevator to 2nd floor.
• Go to Suite 212.
• Check in at main desk.
