Directions:

• From I-205 take Exit 16 for Johnson Creek Blvd.
• Head east up the hill one block.
• Turn right at SE 91st Ave.
• Take the second driveway on your left.
• Enter Mount Scott Professional Plaza II and take the elevator to the 4th floor.
• Turn left for suite 400.
• Check in at main desk at scheduled appointment time.

• From I-205, take the Johnson Creek Blvd – Exit 16.
• Then turn east (uphill) on Johnson Creek Blvd to SE 91st Avenue.
• Please arrive 30 minutes prior to your scheduled appointment time.
• Park by the main entrance and go directly to the Admitting Desk to check in.

From I-84 Eastbound

• Take Exit 3 toward 58th Ave.
• Turn Right onto NE Glisan St.
• Turn Right onto NE 49th Ave.
• Park in the parking structure.
• After parking, please enter the Main Entrance of the hospital and follow the sign to the Main Admitting Desk.
• They will direct you from there.

From I-84 Westbound

• Take Exit 2 toward 43^rd Ave.
• Turn Right onto NE Halsey St.
• Turn Right onto NE 47th Ave.
• Turn Left on NE Glisan.
• Turn Left onto NE 49th Ave
• Park in the parking structure.
• After parking, please enter the Main Entrance of the hospital and follow the sign to the Main Admitting Desk.
• They will direct you from there.

• From US-26 take Exit 64 for 185th Ave.
• At the end of the exit turn left onto NW 185th Ave.
• Turn right at NW Cornell Rd.
• Turn left at NE 106th at the next light.
• Take the first right into parking lot.
• Office is in first building on the left with Unitus.
• Take elevator to 2nd floor.
• Go to Suite 201.
• Check in at main desk.

• Heading South on I-5
• Take Exit 282B toward Charbonneau District
• Turn Left on SW Miley Road at the end of the exit
• Cross over I-5
• Make immediate Left into parking lot after passing on and off ramps

Directions: (Heading North on I-5)

• Heading North on I-5
• Take Exit 282 toward Canby
• Turn Right on SW Miley Road at the end of the exit
• Make immediate Left into parking lot
